Worker killed in fatal fork lift truck accident
A company has been fined £240,000 after a fatal accident could have been prevented.
A 49 year old worker’s death when he was crushed by a two-tonne grain bin fell on top of him.
Forklift Truck Fatal Accident At Work
The employee was visiting a driver to pick up a load. He was helping with the feed operation when the bin fell from a forklift truck.
The employee was standing close to the bottom of the grain elevator ready to pull the release lever to the feed.
When the fork lifter driver picked up the bin he raised the forks around 5 feet for better visibility. The bin then started to move off the forks. The driver shouted warnings to the male who was standing in front of the bin. Unfortunately it was too late and the bin fell on top of him causing the fatal work accident.
The fatal injuries sustained were to his head, neck and chest.
Health and Safety Executive investigations said the firm had no safe work systems, they pleaded guilty to a breach of health and safety law.
