A 52-year-old employee died due to head and chest injuries when a metal frame being loaded onto a lorry trailer collapsed on him at a local transport company he was employed by.
Using two forklift trucks they was moving a large metal frame from the first trailer why a colleague was trying to load it onto a second trailer when the deceased stepped onto the second trailer, the forklift truck carrying the metal frame slipped causing the deceased to become trapped under the metal frame and fatally injured.
