In another deeply distressing example, a mother arrived at hospital in critical condition after losing a significant volume of blood. Miscommunication between ambulance staff and hospital clinicians resulted in fatal delays. Her baby died after being starved of oxygen in the womb. The failings in that case were so stark that legal representatives described them as care so poor that even a bystander would have recognised the danger.
In the south of England, a mother suffered cardiac arrest while in labour due to missed opportunities and poor clinical judgment. She was placed in a coma and only awoke to be told her newborn daughter had not survived. Following her recovery, she discovered several other families had also experienced similar outcomes at the same trust — raising concerns about transparency, reporting, and whether other families have suffered unknowingly from similar failings.
These families are now united in their calls for accountability and lasting reform.
