He became buried in tonnes of grain which acted like quicksand.  He later died.

The deceased, 52-years-old died due to his forklift truck being knocked over by a heavy goods vehicle.

It is believed the deceased works for a well-known distributor when he was working in the factory at the time of the accident which consisted of him picking goods from pallets and putting them into a HGV lorry in order for it to be delivered to supermarkets.

The deceased was in process of loading goods onto the lorry when the truck rolled back causing the deceased to be crushed on the forklift truck which he was operating at the time of the accident.

An investigation took place were it was found that there was no horns fitted into the forklift truck which could of prevented the accident and near misses in the past was not reported which is explained as an “fatal accident at work waiting to happen”.

At the coroners court inquest it was found the deceased died of crush asphyxia.

It is believed the company has been fined £20,000 and ordered to pay the council £199,943 in costs.

A young agency worker died due to an accident at work.

The 20-year-old agency construction worker was one of five other workers to be standing underneath a crane which was carrying a metal cage containing materials for the work which was taking place on the building site when it tragically fell caused the worker to lose his life.

Following a post-mortem examination it was confirmed the worker died of multiple injuries which include multiple fractures to his head and body after the metal cage slipped from the chains connecting it to the crane.

HSE took out an investigation and the company was fined for serious safety failings within the construction site and it was confirmed that the workers was under time pressure and this could of been a possible cause to this tragic accident.

The deceased was taking part in work which consisted of him and a few other colleagues to take out emergency repairs to low overhanging cables and replace them with insulated cables.

It is believed the deceased was up a pole when colleagues decided to cut four wires in which was connected to the pole which then made the pole unstable, snap and fall to the ground resulting in the death of the worker.

An investigation took place in which it was confirmed that there was no notice on the pole in which to advise workers of an ‘unstable’ pole and there was no supervisor supervising the work that was being taken place at the time of the accident.

A post-mortem has revealed that the 28-year-old deceased died due to accidental death when a pole he was strapped to fell and crushed him.

It is believed when the deceased was connecting the red and yellow airline, when the second airline was attached the trailer started to move away and at some point after, he became trapped between the cab and the trailer.

An Inquest Hearing took place where the Coroner ruled that there was no physical witnesses to the accident and officers concluded that the deceased’s death was due to a “driving error”.

It is believed a worker, was taking part in moving around 1,000 tyres on his fork lift truck when it sadly hit a tyre and tipped over and crushed the him, causing the fatal accident at work.

When the accident occurred, it is believed the tyres which the deceased was transporting was stored on the roadway where the accident occurred, which caused an obstruction and increasing the risk of a truck overturning.

A witness at the inquest which occurred advised that he did not see the accident happen but remembered hearing a loud noise and ran down to where it came from when he saw the forklift truck on top of the deceased.

Another forklift truck had to be used to lift the vehicle off him, when emergency services were called but regrettably he was pronounced dead at the scene.

The two employees were taking part in maintenance work on a local bridge when a cable attached to a crane, snapped causing the deaths of the two employees at work.

It is believed the cage was released from the crane and crashed into the river, trapping the men under the surface.

According to a third man, who was in the cage with the two men who sadly lost their lives, they were trapped inside the cage under water for a number of minutes before help arrived.

The surviving man managed to free himself whilst under the surface and swam to safety.
